Serial ECG Analysis

The Serial ECG Analysis tutorial discusses major aspects of this relatively underexplored application of electrocardiology. The tutorial focuses on serial analysis of standard 10-second 12-lead diagnostic resting ECGs. In the outline of the tutorial (Dr. Swenne) the clinical relevance of serial ECG analysis is illustrated. The relatively scarce existing algorithms / computer programs for serial ECG analysis are discussed, including deep learning techniques. The importance of detecting intra-individual ECG changes, even if the ECG remains within normal limits is stressed. ECG changes can imply emerging new pathology, and serial ECG analysis, in selected groups possibly even in the form of periodic ECG screening, can potentially contribute to early diagnosis of such new pathology.

Serial ECG analysis is hampered by various sources of variability of technical and biological origin. An important category of such variability are lead interchanges and electrode misplacements. Dr. Guldenring discusses their diagnostic consequences, and explores automated correction strategies. Another problem in intra-individual comparison of ECGs is caused by the use of Mason-Likar electrode positions instead of the standard electrode positions. Dr. Maan discusses what the diagnostic consequences are, for single as well as for serial ECG analysis. Dr. Swenne discusses biological sources of ECG variability on the short and long term.

Serial ECG analysis is only feasible if the ECGs made in the past are available for comparison when a new ECG in an individual is made. To conclude the tutorial, Mortara Instrument's Barry Brown discusses what the biggest challenges are in having a practical universal ECG comparison solution that is independent of the patient’s location, healthcare provider, device manufacturer, and IT system. Could a central repository be the answer, or could we find ECGs even when they are distributed across healthcare providers and IT systems?  What if Personal Health Records are more universally adopted in the future?
